Despite the availability of free, effective treatment for HIV, more thanone million people die of AIDS every year in sub-Saharan Africa. Increasing antiretroviral treatment coverage is a necessity, from both a global health and a moral perspective (Collier et al. 2015). The need to increase treatment has implications for both the supply side and the demand side of treatment provision.
